PRP- Under-eye Treatments

The most common reasons for consultation for cosmetic treatments around the eye area in our Dallas and Southlake regenerative cosmetic medicine treatment centers are:

  1. Under-Eye Pigmentation or dark circles
  2. Under-eye hollows with or without dark pigmentation
  3. Under-eye fine lines, wrinkles and poor texture
  4. Under-eye wrinkles, loss of elasticity and firmness
  5. Under-eye bags

There are several non-surgical cosmetic treatments such as laser resurfacing, dermal fillers, topical skin lightening cremes for all of the above peri-orbital conditions and each treatment has its own limitations regarding results and possible side effects even in the most experienced hands and with multiple sessions. A year ago we started the Platelet Rich Plasma (PRP)  regenerative treatments for skin rejuvenation and combined the  autologous PRP treatments with the different types of cosmetic treatments available for this area; the CO2 fractional resurfacing laser, Injectable Fractional Radiofrequency, HA dermal fillers,  and Micropen skin rejuvenation to address the peri-orbital cosmetic concerns of our patients with promising results.

General Facts About The PRP Regenerative Cosmetic Treatments:

Here are the some general facts about the PRP treatments for the peri-orbital region:

  1. PRP treatments for the under eye and the upper eyelid area are regenerative treatments which means the PRP injection and application leads to stimulation of your own collagen and elastin and regeneration of new healthy skin tissue for gradual, natural and real results that improve in the 2-3 months following a single treatment.
  2. Two to three treatments, 2-3 months apart are recommended for most people for significant results.
  3. Individual results will vary depending on your body’s response to the PRP induced collagen regeneration.
  4. PRP injection treatments below the skin in the under-eye area will not have the immediate and final results of a dermal filler treatment because the volume added by PRP injection is resorbed by your body within 2-3 days, gradual tissue regeneration begins within days and continues for months, you see the final results 3-4 months after the treatment.
  5. The PRP treatments lead to improvement in all but one aging signs in this area (fine lines, texture, loose skin, loss of volume, deep wrinkles, pigmentation); the presence of large under-eye fatty tissue (under-eye bags) is not treated with PRP, this condition frequently needs surgical removal of the excess fat.
  6. The type of PRP treatment most suitable for you is determined by your presentation of the under-eye condition.

 

Which Cosmetic PRP Under-eye Treatment Is Most Suitable For You?

As mentioned earlier, the type of PRP treatment best suited for individual needs varies with the presentation of under-eye condition. At this time we do the following types of cosmetic PRP treatments for the under-eye and around the eye area:

Improvement Of Under-eye Pigmentation and Dark Circles

For under-eye  and upper eyelid pigmentation, PRP with Micropen and with intradermal PRP injection is an excellent choice and gives dramatic results in most treated individuals. For periorbital pigmentation, dark under-eye circle treatment with PRP we recommend three treatments, spaced 6 weeks apart for best results. The  Periorbital hyperpigmentation also called peripalpebral hyperpigmentation, dark eyelids, dark eye circles, dark circles refers to conditions that present with relative darkness of the periorbital eyelids. It makes people look tired and older than their age. Dark under-eye circles are a common condition and seem to have multifactorial causes that include:

  • Intrinsic Factors- Genetics play a part in presence of dark circles
  • Extrinsic Factors- Exposure to sun, smoking, excessive alcohol use, sleep deprivation, seasonal allergies are some of the extrinsic factors that lead to exacerbation of dark circles

The mechanism of action of PRP is based on the fact that platelets contain many growth factors in their alpha granules. These factors have a well-known role in the process of tissue repair. Thus, the concentration of these substances in injured tissues could be beneficial to providing more agility to the regeneration processes
In general, Platelet-Rich Plasma is a useful and relatively very safe treatment for periorbital hyperpigmentation and may provide an easy solution for a devastating problem. Because of the regenerative nature of this treatment, multiple sessions are needed and individual results vary depending on each person’s healing response.

For our Southlake and Dallas patients with dark under-eye circles and peri-orbital hyper-pigmentation, Dr. Malik injects PRP intradermally (into the thickness of skin), subdermally (just below the skin) and our PRP trained assistant performs a Micropen treatment of this area with topical application of PRP during the micropen treatment.We recommend starting with three sessions, 6-8 weeks apart. Additional treatments may be needed for more advanced pigmentation. Individual results vary.

This treatment includes PRP topical application with Micropen treatment and PRP subdermal and intradermal injection placement. The PRP topical apoplication with the Micropen treatment is done by our PRP trained medical assistant or our aesthetcian and the injections are done by Dr. Malik.

Improvement Of Under-eye Hollows With or Without Dark Pigmentation

For under-eye hollows with or without presence of dark pigmentation, there is more volume loss than what is commonly known as dark circles alone. For best results to reduce under-eye hollws, we have found that PRP treatments alone are not sufficient to generate the lost volume below the skin and most of these individuals need a dermal filler to provide some volume  in addition to PRP which improves the skin firmness and texture for overall natural resulst.  To treat deep under-eye hollows, we add a hyaluronic acid filler with the PRP treatment st the time of the first treatment. The HA filler gives immediate volume which lasts from 6-10 months while the PRP works to stimulate collagen and elastic for skin thickness, firmness and texture improvement. The advantage of adding PRP is a more natural appearing fullness with less HA due to the added benefit of dermal thickness and firmness. Three sessions are recommended for best results in most individuals, spaced 6-8 weeks apart, additional treatments with or without HA may be needed for more advanced loss of volume. Individual results vary.

The first treatment includes:

  • PRP topical with Micropen done by one our skin treatment professionals
  • PRP intradermal and subdermal injections by Dr. Malik or Lisa Artman, our certifed dermal injector.
  • One 1cc syringe of Belotero Balance HA dermal filler for under-eye placement by Dr. Malik

The second and third treatments include:

  • PRP topical application with Micropen by our skin care professional
  • PRP under-eye injection placement

Under eye fine lines, wrinkles and poor texture of moderate severity are best treated with DOT  CO2  fractional laser resurfacing in our Dallas office.

The CO2 fractional laser provides skin resurfacing. DOT Therapy (Dermal Optical Thermolysis) is a minimally invasive, non-surgical way to transform skin and enhance its overall appearance. The CO2 fractional laser precisely creates thousands of microscopic holes (DOT’s) in your skin, which induce immediate skin tightening and stimulate new collagen growth. Patients are often amazed at how tight their skin feels immediately after the procedure. DOT Therapy is designed to improve the skin’s texture and tone and to fight fine lines, deep wrinkles, acne and other scars.

 

The role of PRP in skin surfacing treatments is to provide platelet growth factors to the freshly created laser induced micro-injuries for quicker healing, shorter downtime and enhanced collagen/elastic synthesis which eventually translates into better results from your laser skin resurfacing treatments in Dallas . Three sessions are recommended for best results in most individuals, treatments are done 4-6 weeks apart, additional treatments may be needed for more advanced aging. Individual results vary. The CO2 laser resurfacing is available in our Dallas office only. For the under-eye area and upper eyelids, it is best to use lower energy and lesser depth for the laser settings, applied multiple times a few weeks apart for safer induction of collagen in this delicate area. Lower energy and less depth also means very little discomfort and very little downtime. Most people experience mild redness and mild swelling for 2-4 days. PRP injections may lead to occasional bruising.

Each treatment includes skin resurfacing with the DOT CO2 laser in Dallas. PRP topical application and PRP intradermal and subdermal injection by Dr. Malik.

Improvement Of Under-eye Loose Skin. Loss Of  Elasticity And Firmness

If the primary concern for the under-eye area is loss of skin elasticity with loose skin and sagging of the under eye area then a treatment aimed at improving skin elasticity and firmness is the most suitable option. At this time injectable fractional radiofrequency treatments are the best option for non-surgical skin tightening. (such as Infini, Fractora, Scarlet etc).

The injectable RF or the Microneedle Fractional Radio frequency (MFR) treatment, delivers radio frequency energy via microneedles below the skin surface in multiple minuscule beams leading to “fractional” heat zones, leaving normal tissue in between the heated zones for shorter recovery time. This treatment helps generate collagen in two ways; The microneedles cause mico-injuries in the deeper dermis which your body heals with collagen production. The fractional radiofrequency heats up the deeper dermis in 360 degrees around each microneedle tip for further collagen tightening and synthesis. Our injectable RF system uses gold plated and insulated needles for minimum injury to skin surface for little or no downtime.

We combine PRP with the injectable RF or Microneedle Fractional Radiofrequency treatments for the same reason as all the other cosmetic treatments the results of which depend on your body’s healing ability. Your PRP’s growth factor infusion of the area treated with MFR leads to quicker recovery and enhanced collagen/elastin synthesis for better results.

The PRP + MFR treatment is done by our PRP trained medical assistant. The PRP intradermal and subdermal injections are performed by Dr. Malik or Lisa Artman, our certified injector.

Each treatment includes the microneedle fractional radiofrequency -MFR, multiple pass therapy done by our PRP trained medical assistant, one PRP kit which is applied topically by the PRP trained assistant at the time of the MFR and PRP dermal injections into the skin and below the skin by Dr. Malik or Lisa Artman. Three sessions are recommended for best results in most individuals, additional treatments may be needed for more advanced aging. Individual results vary.

Not everyone falls into one or the other classification of under-eye aging. For many individuals a combination of treatment can work best.
